Meeting registration information must be provided using the following form. You may download the form and print it with Adobe Acrobat (see below). Meeting registration must be made with the registration form together with $120 in US funds in order to be registered. The address for mailing is on the form. If a letter of invitation is required, please contact the meeting organizer, Dr. Roy Van Driesche.
